You will need to complete your consultation form prior to arrival. For facials and Lip Magic appointments, you’ll need to upload photos to these forms. It is best to either have someone take photos of you or use your phone’s back camera. Make sure your photos are well lit, avoid flash, and take them on a plain background. Failure to submit your forms prior to your appointment may result in not receiving your full desired treatment. Click below to access the right form(s).
